• Letter of referral from your General Practitioner
  
• Any results relevant to your consultation such as blood test results, ultrasound results etc.
  
• Your completed New Patient Information Form [PDF]. This form can be filled electronically using the latest free version of Adobe Reader.
  
• Your Medicare card and Private Health Insurance details (such as name of fund, level of cover and membership number). It is recommended that you also bring your fund membership card to your first consultation.
